Pest Control Operator's (PCO) report
This form is an example of a Pest Control Operator's (PCO) report. The form
contains information related to the fumigant name, amount used, treatment
duration and which areas were treated on a vessel. This form also serves as a
Certificate/Declaration that the vessel is gas free and safe to enter and/or
load grain commodities.
Some of the information the fumigator (PCO) will need to fill out on the form
are:
- Vessel name and the treatment date
- Hold #
- The total volume of the hold (cubic capacity)
- Whether the upper (U) or lower (L) deck of the hold was treated, or both
- The time treatment of the hold was initiated (Gas in:)
- The time treatment of the hold ended and gas was exhausted (Gas opened:)
- The time at which the hold was declared gas free (Gas free:)
- Whether or not additional treatment was required (Extra gas added:)
- The temperature on the deck:
- The temperature in the hold
- The name of the chemical or fumigant used in the hold
- The concentration of the fumigant used expressed as grams of fumigant per
cubic meter of the hold
- The total amount of extra fumigant used (Total extra gas used:)
- The total amount of fumigant used: (Total gas used:)
- The total volume of the hold (total capacity)
- A gas free certification declaration including the date and time this was
achieved and signing blocks for the PCO and the Ship's officer.
