Part D: Approval (To be completed by CFIA)

The Application to Register in the Emerald Ash Borer Approved Facility Compliance Program must be completed by facilities interested in becoming approved under the Emerald Ash Borer Approved Facility Compliance Program (EABAFCP). The application is divided in 4 parts. The part A, entitled Applicant/Permit Holder Application, the part B, entitled Facility Consent, and the part C, entitled Declaration, must be completed by the facility. The part D, entitled Approval, is to be completed by the CFIA to document when the facility satisfactorily completes their quality manual and their evaluation audit and if the facility is given final approval by the CFIA under the EABAFCP.

A CFIA Use Only section is used by the CFIA Regional Program Officer to inform the Area Program Specialist that a facility has completed their application, manual review, and evaluation audit requirements of the EABAFCP and should be considered for approval under the EABAFCP. If the Area Program Specialist agrees, they then sign the Application form and send it to the Emerald Ash Borer lead in Headquarters and an EABAFCP registration number is issued. Areas to complete include under which type of facility the facility is approved (sawmill, firewood producer (domestic only), disposal or other (need to specify), comments, names of auditors, signatures of auditors and date of manual approval, auditors signature and date of evaluation audit approval, name of CFIA Regional Program Officer responsible for EABAFCP, Regional Program Officer signature and date, name of CFIA Area Program Specialist responsible for EABAFCP, Area Program Specialist signature and date, facility's EABAFCP registration number.
