Audit Report

The Audit Report form must be completed by the CFIA Auditor following a facility audit for the EABAFCP. Sections A, (Facilty Information), B (Non-conformances), C (Results of Audit) and E (Approval of Audit) are to be completed by the CFIA. Section D (Acknowledgement of report by facility) must be completed by the facility. The Audit Report contains the following information fields:

Non-conformances information is required, including the following:

A field is provided to input the CAR number for all CAR for which a follow-up action is required. The CFIA Auditor must check if the CAR is for a minor, a major or a critical non-conformance. There is also a field for the date by which the action is required. (10)

Information on the result of the audit is required, includind the following:
