Corrective Action Request (CAR)

The Corrective Action Request (CAR) form must be completed when a non-conformance has been identified and corrective actions are necessary. The form includes 4 sections. Sections A (Facility Information), B (Description of Non-Conformance & Related Observations, and D (Verification of Corrective Action) must be completed by the CFIA. Section C (Description of Corretive Actions) must be completed by the facility, with the exception of the Approval of Part C, which must be completed by the CFIA. The CAR form contains the following information fields:

Title of section A: Facility Information (3)

Facility information is required, including the following:

Facility name and address (4)

Contact name & information (5)

Title of section B: Description of Non-conformance & Related Observations (6)

Information required in that section includes:

Date of audit (7)

Non-Conformance Type, with check boxes for options (Critical, Major, Minor or Other- to specify) (8)

A field to describe the non-conformance (9)

Auditor's printed name & signature (10)

Date of signature (11)

Title of section C: Description of Corrective Actions (12)

The first portion on this section includes a field provided for the facility to describe the corrective actions taken, followed by fields of date for completion of corrective action as well as Facility representative's signature and date (13)

The second portion of this section is for the Approval of the corrective actions, with fileds for the Auditor's printed name & signature and date. (14)

Title of section D: Verification of Corrective Action (15)

This section includes check boxes to specify if the corrective actions are acceptable or not acceptable, a field for follow-up visit findings and/or additonal comments, check boxes to say if the CAR is closed (yes or no), the Auditor's printed name & signature and date. (16)
