APPENDIX III: WEIGHTS FOR SUBMITTED SAMPLES
At a minimum, the quantity specified for the test(s) requested must be submitted for testing.
Where the composite sample submitted to the laboratory is for 2 or more of the following tests in this table, the size of the submitted sample shall be at a minimum a total of the quantities stated for each test.

APPENDIX IV: SEED PROGRAM CODES
| Code | Explanation |

| AEP | Authorized Exporter Pre-accreditation Samples. Samples drawn by a company as part of the pre-accreditation exporter check sampling and testing program to check commercial lab results and to provide a check sample to CFIA. |
| AEM | Authorized Exporter Monitoring Samples. Samples drawn by an inspector as part of the post-accreditation monitoring program of an Authorized Exporter (AE). These samples are drawn from the same seed lots as are sampled by the company under the AEX program. |
| AEX | Authorized Exporter Samples. Samples drawn by a company under the AE program. Samples from 10% of the seed lots exported by a specific company are submitted to CFIA under CFIA request. |
| BRD | Breeder. A sample received from a plant breeder for testing. This is a service provided to plant breeders. Not usually submitted by CFIA inspectors. |
| COM | Compliance / Complaint. A sample drawn by a CFIA inspector in response to a complaint, or to follow-up corrective actions or when there is a suspected non-conformance, or to verify compliance. |
| EXP | Export. A sample drawn by an official seed sampler for the purpose of obtaining an ISTA International Seed Lot Certificate (Orange or Green). Samples submitted for Purity, Germination, Purity of Variety and Disease testing for export certification are to be submitted under the EXP code, with the appropriate test requests indicated under “Analysis requested”. |
| IDS | Identification Sample. A sample consisting of one or more seeds or plants submitted for identification. |
| INQ | Inquiry sample. General code for samples for which there is no other code. INQ samples may be submitted by CFIA inspectors or other parties. |
| INV | Investigation. A sample drawn by a CFIA inspector once a non-compliance has been verified and an offence has occurred in order to recommend further compliance measures or enforcement action. Sample handling must be performed according to chain of evidence rules. May lead to prosecution. Sampler should notify the lab prior to submitting. |
| IOM | Import Official Monitoring. A sample taken by an inspector from a seed lot imported by a non-authorized importer or an Authorized Importer. These samples are taken to verify that the seed meets minimum grade table requirements (e.g. Common No. 2). Samples taken to verify that seed meets grading/ labelling requirements are taken under the MPI code. |
| MAI | Multiplication Agreement - Import. A sample submitted for variety verification by a company who has multiplied the seed outside the country under a multiplication agreement. These samples are not normally drawn by CFIA inspection staff. |
| MPI | Marketplace Monitoring -Imported Seed. A sample drawn by an inspector from an imported seed lot once imported seed is offered for sale. This sample is taken to verify the accuracy of the grading/ labelling of seed. |
| MPN | Marketplace Monitoring - Non-Pedigreed. A sample drawn by a CFIA inspector for confirmation of grade of domestically produced non-pedigreed seed which is being offered for sale in the marketplace. |
| MPP | Marketplace Monitoring - Pedigreed. A sample drawn by a CFIA inspector for confirmation of grade of domestically produced pedigreed seed which is being offered for sale in the marketplace. |
| NFC | Not Finally Certified. A sample drawn by a CFIA inspector from a seed lot that is being exported under the OECD Seed Schemes. To be stored for possible POV testing. Not tested for purity or germination. |
| OSS | Official Seed Sealing. Samples drawn by a CFIA inspector to determine the grade of a seed lot for labelling purposes. Rarely used, and only when an establishment does not have access to a private sector sampler. |
| PHS | Plant Health Sample. A sample drawn by a CFIA inspector to determine weed and/or soil content of imported grain or seed. High priority samples are from lots under detention pending test results. Low priority samples are from lots released into Canada but sampled as part of a survey. |
| PHY | Phytosanitary. A sample drawn by a CFIA inspector for tests in support of a phytosanitary certificate. |
| POV | Purity of Variety. Samples drawn for OECD post control or other variety testing. |
NOTE: There are no requirements for IDS, POV, BRD, PHYor PHS samples to be submitted by “official seed samplers”. In the case of INQ samples, if submitted by non-CFIA personnel, or by CFIA personnel for information purposes only, the samples need not be submitted by official seed samplers.

Instructions for Completion of the Sample Submission Form (CFIA/ACIA 5160)

The following instructions also apply:
Where the sampler makes an error in recording information on the Sample Submission Form (hard copy), each error must be crossed out (not erased, made illegible, no white-out) and the correct information written alongside. All corrections must be initialled and dated. All information must be written in ink.
When sampling for the issuance of an ISTA Certificate, the sampler must state on the LSTS form what method of sealing was used i.e. OECD stitched label, metal seal.
Where the sampler is submitting a sample that is treated, the name of the pest control product must be stated.
Where the sampler is submitting a sample that is inoculated, the name of the inoculant product must be stated.
Where the trier size or methods for sampling deviate from those stated in this Specific Work Instruction (SWI), the reason must be documented on or accompany the Sample Submission Form.
Where the sample submitted is a seed mixture, the sampler must state the name and percentage of each species comprising the mixture on the Sample Submission Form.
When the sampler is submitting a marketplace surveillance sample, the information as described in Section 12.1.1 - Market Place Surveillance - shall be stated on the Sample Submission Form.
When the sampler has mixed and divided the sample, this shall be stated on the Sample Submission Form.
When submitting samples for Plant Health (Seed Program Code PHY or PHS), the sampler must state on the Sample Submission Form for what species the sample must be examined, or the specifications must be attached to the Sample Submission Form.
