Submission Completeness Checklist

  • A letter explaining the intent of the submission. The letter should indicate the amendment is either one or more of the following (Any changes not listed below will be considered a Major Amendment):
    • name or address of the registrant and/or manufacturer;
    • colour or format of the label;
    • product name and/or brand; and/or
    • net weight.
  • A copy of the proposed marketplace label.
  • A completed Fertilizer and Supplement Registration Application - PDF (642 kb) form including:
    • Constituent materials: identification and description of all materials used in the production of the end-product, the source and proportion of these materials (include all formulants: dust controls or anticaking agents, etc.).
    • Other Qualities: this includes physical characteristics (e.g. colour, granular/liquid,etc.), description of the manufacturing process, and size guide number (SGN) for blending products.
  • Registration Fee ($50.00 + HST).
  • Designation of signing authority T4-95 or written indication that signing authority is unchanged from previously provided documentation.
  • If applicant lives outside Canada, Declaration of the Canadian Agent Form - PDF (65 kb) or written indication that Canadian Agent is unchanged from previously provided documentation.

Minor amendments may be made at the time of re-registration at no additional cost. The minor amendment fee is applicable only when the product proponent wishes to amend a product between registration periods. The processing of a minor amendment in between registration periods does not extend the duration of the product registration.
